Corneal Disorders: Symptoms, Causes & Treatment

Overview

The cornea is the transparent, dome-shaped front surface of the eye that focuses most of the light entering it. Because it is directly exposed to the environment, it is vulnerable to injury, infection, dryness and a range of inherited and acquired conditions.

Corneal disorders range from minor surface scratches that heal quickly to sight-threatening infections (corneal ulcers) and progressive structural conditions such as keratoconus. The common thread is that the cornea's clarity is essential for sharp vision, so even small areas of scarring or swelling can affect sight.

Symptoms

• Eye pain, redness and watering
• Foreign body sensation or a feeling of grit in the eye
• Blurred or reduced vision
• Sensitivity to light (photophobia)
• Visible white or greyish spot on the cornea (in infections or ulcers)
• Excessive tearing or discharge

Causes

• Bacterial, viral or fungal corneal infections, often related to contact lens misuse or minor trauma
• Corneal abrasion or injury from a foreign body, fingernail scratch or chemical splash
• Severe or chronic dry eye affecting the corneal surface
• Inherited corneal dystrophies
• Keratoconus (progressive thinning and bulging of the cornea)
• Previous eye surgery or long-term contact lens wear

Risk Factors

• Contact lens wear, especially overnight or poor hygiene
• Eye injury or exposure to dust, chemicals or foreign particles at work
• Chronic dry eye or eyelid conditions
• History of eye surgery
• Vigorous eye rubbing, particularly in allergy or keratoconus-prone individuals
• Diabetes, which can slow corneal healing

When to See a Doctor

See an ophthalmologist urgently — ideally the same day — for eye pain with redness, a visible white spot on the cornea, sudden reduced vision, or discomfort following contact lens use, injury or a chemical splash.

Even minor-seeming corneal scratches should be examined, since untreated corneal injuries and infections can progress rapidly and lead to scarring that affects vision.

Diagnosis

Corneal disorders are diagnosed through careful slit lamp examination, often supplemented by dye staining and imaging to assess the exact location, depth and cause of the problem.

Tests

• Slit lamp examination with fluorescein dye staining to detect surface defects and ulcers
• Remidio anterior segment imaging for detailed documentation of corneal findings
• Corneal topography, available on a rental basis, to map corneal shape (important in keratoconus and before certain treatments)
• Microbiological evaluation (scraping/culture) for suspected infective corneal ulcers, arranged as needed

Treatment Options

Treatment depends entirely on the underlying cause and severity, ranging from simple protective measures to intensive medical therapy or surgery.

• Antibiotic, antiviral or antifungal eye drops for infective corneal ulcers, with close monitoring of response
• Lubricating drops and a protective eye patch or bandage contact lens for corneal abrasions
• Treatment of underlying dry eye or eyelid disease contributing to corneal surface damage

• Referral for corneal transplantation in select advanced cases where medical treatment is not sufficient

Expected Outcomes
